Just fold down the flaps of a 24x20x4 on top of a scrap or flat of 24x20. Or if you really want to waste money use two 24x20x4, lid style.

Probably not, FR is generally not a good deal. To get around this issue I'll just send you a label and you can send the entire box full of them to me. Then you don't need to worry about all those shipping headaches.

Don't do this. You can do up to 50% if you wanted as it is materially different than what you sent. I would do 20% plus cost of return label.

Not necessarily, but probably. They are being super stingy now with reimbursements, etc., and since you didn't respond in time technically they are off the hook. Try another appeal to two, making sure to highlight the fact that you proactively tried to alert them. If that doesn't do it send an email to Jeff@. With a little persistence you may be able to recover the funds.
